Terms of Clearance:
Clearance granted for a one year period consistent with the HRSA memo on July 20, 2001, which includes edits to Table 2, Services Offered and Delivery Method. After one year HRSA will re-examine this collection and analyze the scope and depth of data received involving CAM services as well as any 'other' category data. HRSA will then re-assess the necessity of this collection and its effectiveness and report back to OMB.

Abstract:
The Uniform Data System (UDS) is the Bureau of Primary Health Care's annual reporting system for grantees. The UDS includes reporting requirements for grantees of the following programs: Community Health Center, Migrant Health Center, Health Care for the Homeless, Outreach and primary Health Services for Homeless Children, and Health Services for Residents of Public Housing.
