Abstract:
The information collected on form SSA-7050 is used by the Social Security Administration (SSA) to identify the requestor, to define the earnings information being requested, and to inform the requester of the fee for such information. Based on the information provided, SSA produces the requested statement. The respondents are individuals and organizations that use this form to request statements of earnings from SSA.
