Abstract:
The information collected on Form SSA-3441 is used by the Social Security Administration (SSA) to obtain a statement from the claimant of his/her physical, emotional and/or mental conditions as perceived by the claimant. The form also elicits additional sources of medical and vocational evidence that were not considered in the initial determination.
