Terms of Clearance: Approved. It is noted that this package has been in violation of the PRA since 12/2000. HUD shall use the procedures detailed by senior policy officials to eliminate future violations. This violation should be reported in the next ICB. Upon reprinting of the form, HUD will change its procedures to reduce the number of copies required (the form is now required to be filed in quadruplicate) and will also modify the form to better explain what values are to be placed in columns A, B, and C.
