Abstract:
SSA uses the information on Form SSA-1696-U4 to verify the applicant's appointment of a representative. It allows SSA to inform the representative of issues that affects the applicant's claim. The respondents are applicants who notify SSA that they have appointed a representative and the individual and/or organization that represents them.
