Abstract:
The information is needed to identify individuals who request access to their records, to designate an individual to receive and rview a record holder's sensitive medical records, to correct or amend records, to obtain consent from an individual to release their records to others, to facilitate the release of the information under the Freedom of Information Act and to grant waivers or reduction of fees for records. Respondents are individuals requesting any one or more of the above actions.

The following is a summary of the topics, regarding the proposed collection of information, that the certification covers:
