Abstract:
The information collection requirement is necessary to ensure that applicants for CHAMPUS-Provider Status meet the conditions for authorization as a CHAMPUS Corporate Services Provider. The collected nformation will be used by CHAMPUS contractors to process claims and verify authorization status.
