Abstract:
Applicants must submit NRC Form 313 and 313A to obtain a specific license to possess, use, or distribute byproduct or source material. The information is reviewed by the NRC to determine whether the applicant is qualified by training and experience, an and has equipment, facilities, and procedures which are subject to protect the public health and safety, and minimize danger to life or property.

Short Statement:
Since the last renewal clearance request which was approved on 10/19/05, NRC replaced the currently used NRC Form 313A, a single form with a series of six distinct forms for medical use training and experience and preceptor attestation. The replacement forms are easier to complete and provide clearer guidance for each licensee type to complete.

On behalf of this Federal agency, I certify that the collection of information encompassed by this request complies with 5 CFR 1320.9 and the related provisions of 5 CFR 1320.8(b)(3).
