Abstract:
The three FCC forms (FCC Forms 479, 486, and 500) serve the functions of the Universal Service Schools and Libraries Support Mechanism (47 U.S.C. Section 254, et seq.). They are used at the point where services provided pursuant to the Program are implemented; or about to be implemented and are a necessary prerequisite to the distribution of payments under the Program. The forms ensure that congressional mandates regarding access to the Internet by minors are met.

Short Statement:
The Commission has revised this collection because the FCC Form 486 contains a new certification that certain steps have been taken prior to the commencement of service. This change was adopted in the Fifth Report and Order in FCC 04-190, WC Docket No. 02-6. The Commission has reduced the burden for this collection by -12,500 hours. The reason for the adjusted burden is because there are 5,000 less hours for the FCC Form 479 reported since the 2004 submission to OMB; the burden hours for FCC Form 486 remains unchanged; and the burden hours have been reduced by 7,500 hours due less respondents (from 10,000 respondents to 5,000).
