Abstract:
The Telecommunications Act of 1996 contemplates that discounts on eligible services shall be provided to schools and libraries, and that service providers shall seek reimbursement to the amount of the discounts. FCC Forms 473 and 474 facilitate the reimbursement process. FCC Form 472 allows providers to confirm that they are actually providing the discounted services to eligible entities.

Short Statement:
The Commission has adjusted the burden estimates for this collection. The Commission is submitting this ICR as a revision because the FCC Form 473 contains new certifications that the service provider has complied with the competitive bidding requirements, pursuant to the Fifth Report and Order (FCC 04-190). In addition, to reduce confusion, the FCC Form 473 will contain information about one SPIN, rather than multiple SPINs. The burden hours have been updated to reflect more accurate estimates recently obtained regarding participation in the Program.
