Terms of Clearance:
DOD needs to re-submit these forms before July 2010 at the 30 day notice stage. Please note the additional terms of clearance. DOD needs to do a better job providing instructions on the form itself -- dates are to be filled as dd/mm/yyyy, does the form need to be typed, all phone numbers need to include areas codes, do all questions need to be answered, what happens if people need to make changes to the form after they sign it, what happens if some believes they need additional space, etc? The form needs to also include the following information. 1. Purpose of the form. 2. What is the authority to collection this information? What is DOD's authority to ask for SSNs? 3. What is the process the applicant will go through -- this is less than a paragraph. 4. Who makes the final determination on eligibility -- and will the applicant have the opportunity to explain any information before a final decision is made? 5. Are there any penalties for inaccurate or false statements? 6. Disclosure of the information DOD needs to describe who this information is going to be shared with and the privacy act routine uses 7. Authorization for release of the information -- this needs to include a signature -- as well as how long the authorization is valid (probably not more than a year). On the West Point Admissions Request and the candidate address form Please have them put the purpose of the form first. Also, can we write the privacy act and disclosure sections a bit less abruptly? Prospectus There needs to be a purpose. Also, why do they need to collect a SSN? Im not comfortable with the SSN being so visible on the form. Admissions Participant Personnel Information needs a purposes and some other information describing the type of information folks will need to put it -- what are remarks supposed to be? For item 21 race will need some additional explanation what are the options? For references, dont you want phone numbers? Wouldnt you want email addresses for folks as well as for the references? Also, for items 12 wouldnt you want to give them more space or an extra sheet of paper if they had more than one school? Also, wouldnt you want them to sign this saying this information is correct to the best of their knowledge and also letting them know what the appeal process is if any for any inaccurate information? Also, how long is the information val Candidate Questionnaire form purpose needs to be written as well as a privacy act and disclosure release. The race and ethnicity group section will also need to be re-written to conform to OMB guidance. Please see http://www.whitehouse.gov/omb/fedreg_1997standards/. In particular, there should not be a category for other or unknown in the race groups and both the form and the instructions need to allow for more one race grouping to be picked.
