Abstract:
Surviving spouses, spouses, and children of veterans send VA this form to apply for a change of their program of education and/or a new place of training under 38 U.S.C. chapter 35. VA uses the information on the form to determine the current eligibility of the claimant for educational assistance for the new program of education or new place of training.

On behalf of this Federal agency, I certify that the collection of information encompassed by this request complies with 5 CFR 1320.9 and the related provisions of 5 CFR 1320.8(b)(3).
