The U.S. Department of Education Supplemental Information form for the SF-424 is used together with the SF-424, Application for Federal Assistance. The Supplemental Information form procures several necessary data elements and questions that are not included on the SF-424.

There is an adjustment decrease in both the number of respondents and the number of burden hours. The number of respondents for this collection is 19,000 compared to the current inventory of 26,000; the burden hours total 6270 compared to the current inventory of 7860. The adjustment decrease in both the number of respondents and the number of burden hours is due to the deletion of the formula programs from this collection since the formula programs do not use the SF 424 or the ED Supplemental Information form.

On behalf of this Federal agency, I certify that the collection of information encompassed by this request complies with 5 CFR 1320.9 and the related provisions of 5 CFR 1320.8(b)(3).
