Abstract:
The Commission is submitting this revision to obtain OMB approval for a slight modification to all the FCC forms (FCC Forms 465, 466, 466-A and 467) to add a checkbox for funding year 2009. The Commission has also created templates, samples and spreadsheets provided to program participants to facilitate the reporting, recordkeeping and/or third party disclosure requirements under this collection of information. Attachment 9 was previously approved by the OMB but is included in this submission so that all the documents are together and can be viewed for convenience. Also, attachment 2, Invoice Template is in two parts; one for the primary program and the other for the pilot program.
