This collection was in use without a valid OMB control number, which is a violation of the PRA. OMB recommends that the agency review its internal procedures to ensure that requests to create, extend, change, or discontinue collections are submitted on time to OMB, after complying with the PRA's other requirements. This collection was submitted as a "reinstatement with change," however the agency advises there is no change to this collection. OMB is approving this collection for 18 months while the agency continues to make progress towards making it fully electronic.
