The adjustments are an increase in the number of respondents to 4,400 and a reduction in the time it takes to complete the form from 45 minutes to 20 minutes. The total burden hours increased to 1,467 hours. The adjustments are based on forms submitted during FY 2011.

On behalf of this Federal agency, I certify that the collection of information encompassed by this request complies with 5 CFR 1320.9 and the related provisions of 5 CFR 1320.8(b)(3).
