Abstract:
The Telecommunications Act of 1996 contemplates that discounts on eligible services shall be provided to schools and libraries, and that service providers shall seek reimbursement to the amount of the discounts. FCC Forms 473 and 474 facilitate the reimbursement process. FCC Form 472 allows providers to confirm that they are actually providing the discounted services to eligible entities.

Short Statement:
The Commission made changes to FCC Forms 472, 473, and 474 and their associated instructions to ensure consistency with the FCC's rules and improve the clarity of the instructions. This revision also updates the respondent estimates and burden hours for the FCC Forms 472, 473 and 474 to reflect more accurate information recently obtained from USAC regarding participationn in the E-rate program. The burden increase of 15,015 hours is due to more entities applying for discounts for eligible services.
