Abstract:
This information collection request (ICR) form will be used to more efficiently collect the information required by FMCSA to process name and addresses and reinstatements of operating authority requests. This multi-purpose form will simplify the process of gathering the information needed to process the entities' requests in a timely manner, with the least amount of effort for all parties involved.
