Short Statement: This is a revision of a currently approved collection; the current burden inventory for this information collection is 73 burden hours. This revision seeks to request an estimated annual burden hours of 870. This is an increase of 797 burden hours. This is a direct result of the increased number of respondents filling out this form (from 435 to 5,220 and increase of 4,785 users). Additionally, FNS has increased the frequency of responses for State Agencies from 1 to 2 times per year. Although the time to fill out the form remains unchanged, FNS has revised the FNS-674 to include additional requirements needed to identify users and verify completion of Information Security Awareness (ISA) Privacy Act Training prior to processing the form.
