Short Statement: Twenty-seven previously approved forms were updated based on user feedback and clarifications in surveillance definitions. Two forms were removed from the package as they were not built into NHSN. Attachment G-1(Crosswalk for Supporting Documentation and Corresponding Data Collection Forms) will provide a table indicating which documents supports what data collection instrument under Attachment C.
