Short Statement: This information collection is necessary to implement NASA policy NPR 8621.1, Mishaps and Close Call Reporting Investigating and Record Keeping to protect the Agency workforce and assets from harm. Mishap reporting enables NASA to conduct thorough investigations to determine causes, mitigate hazards, develop lessons learned, evaluate Agency trends, improve safety and reduce risk.
