Short Statement:
The form is used to request a substitute savings bonds purchased in paper form. The sale of paper savings bonds ceased in December 2012 unless a Series I Bond is purchased with a tax refund. The decline in the use of the form is proportionate to the amount of paper savings bonds issued. The reduction of 2,338 burden hours is a program change due to agency discretion for a total of 167 hours requested.

On behalf of this Federal agency, I certify that the collection of information encompassed by this request complies with 5 CFR 1320.9 and the related provisions of 5 CFR 1320.8(b)(3).
