Abstract:
The Centers for Disease Control and Prevention (CDC) seeks OMB approval for a Non-Substantive Change Request to add a form (Attachment G - PERFORMANCE PROGRESS and MONITORING REPORT – OFR Risk Questionnaire) to 0920-1132 (PPMR). - Addition of this form will allow CDC to collect data for internal use that will aid in the assessment of an applicant’s risk level using a standard, objectively-applied scoring rubric. This will also help CDC to gain a better understanding of its grant applicants, so that it can better serve those who receive awards. No significant change in Burden Hours is expected to result from addition of this form.
