This collection is approved for a period of six months due to noncompliance with previous terms of clearance. These previous terms of clearance are included below: "This collection has been approved for a period of 18 months. Before resubmission, the agency shall create PDF-Fillable versions of all the forms in this collection. Further, in conjunction with OMB 2120-0729, the agency shall evaluate allowing the use of e-signatures in accordance with GPEA. The agency shall provide a detailed report to OMB with the results of their evaluation and specific next steps for compliance."

On behalf of this Federal agency, I certify that the collection of information encompassed by this request complies with 5 CFR 1320.9 and the related provisions of 5 CFR 1320.8(b)(3).
