Abstract:
The CDC Oral Health Management Information System helps state awardees organize their oral health program information and efficiently generate progress reports. CDC requests a reinstatement with change to continue collecting information for two more years to allow CDC to collect final progress and evaluation report data. Changes include minor edits to the user interface and the removal of the initial system information population.

The following is a summary of the topics, regarding the proposed collection of information, that the certification covers:
