Abstract:
The Board uses the FR 1400 to assist in the competitive process of soliciting proposals from vendors of goods and services. The FR 1400 family of reports is comprised of the: Vendor Database (FR 1400A) Solicitation Package (Solicitation, Offer, and Award Form; Supplier Information Form, Past Performance Data Sheet; and Past Performance Questionnaire (FR 1400B) Vendor Risk Management Offeror Questionnaire (FR 1400C) Subcontracting Report (FR 1400D)

On behalf of this Federal agency, I certify that the collection of information encompassed by this request complies with 5 CFR 1320.9 and the related provisions of 5 CFR 1320.8(b)(3).
