Short Statement: This is an extension with a revision of a previously approved information collection. There was an decrease of 14 respondents from the previous collection of 125 respondents to 111 for this collection. The current burden hours for this collection is 3,380 which is a decrease of 308 from 3,688 burden hours in the previous collection. The change is due to the year to year program utilization fluctuation as well as the number of qualified applications received furing the year. Additionally, changes were needed for the forms used in calculations due to changes in enabling legislature and procedures.
