As mentioned above in the submission summary at the beginning of this document, this is a request for an extension with change of the current approval for this collection of information. There was one (1) adjustment that decreased the burden by four (4) hours and 15 responses from the last approved submission (as shown in the table provided in the answer to question number 15 of the attached Supporting Justification). The adjustments under section 216.13 then decreased the burden by four (4) hours and by 15 responses from the last approved submission. The current OMB inventory shows a total burden of 20 hours and 72 responses, while the present submission exhibits a total burden of 16 hours and 57 responses. Hence, there is a total decrease of four (4) hours and 15 responses from the last approved submission. There is no change in costs to respondents from the last approved submission.
