In accordance with 5 CFR 1320, this collection is approved for use as a common form for three years. As the host agency, Forest Service is responsible for notifying sister agencies, whose use of and burden for the SF-299 form was previously included in the Forest Service collection under OMB Control Number 0596-0082, that the SF-299 form under that collection has expired. Forest Service shall also notify these agencies that they must now apply to use the SF-299 form approved with this collection, by reporting the burden associated with their use of the form through a Request for Common Form, in order for their use of this newly approved form to be valid under the PRA, and to update all existing web pages with links to the SF-299 form. Finally, Forest Service has agreed to withdraw the incorrect submission with ICR Reference No. 201907-0596-001.

Description/Changes for Common Form Usage:
Executive Order 13821 Streamlining and Expediting Requests to Locate Broadband Facilities in Rural America directed GSA to coordinate with the heads of Federal property managing agencies to evaluate the effectiveness of GSA’s existing Common Form Application and determine whether any revisions to the GSA Common Form Application were appropriate. This effort identified that most agencies were using the SF-299 as the application for locating or modifying communications facilities or uses on federal property, not GSA’s Common Form Application. The agencies collectively decided that the SF-299, with minor modifications, would be adopted as the common form. Additionally, Section 606(b)(2), division P, Consolidated Appropriations Act, 2018, also known as the Making Opportunities for Broadband Investment and Limiting Excessive and Needless Obstacles to Wireless Act (MOBILE NOW Act), requires GSA to develop a common form for applications for easements, rights-of-way, and leases. The common form is to be used by all executive agencies and applicants with respect to the buildings or other property of each agency.
