Short Statement: We are increasing the burden information to accommodate new survey which we expect to use with millions of respondents across the nation who use the in-office kiosks in our field offices for their appointments. When we last obtained OMB approval for this ICR we did not anticipate any high volume qualitative surveys; however, this new survey for respondents using in-office kiosks is high volume, and requires an increase in the burden information.
