Abstract:
The request for Approval of Qualification Training and Approval of Contractor Flight Crewmember forms will provide the Government information on contractor personnel. This information is used to initiate qualification training and to approve contractor personnel to fly government aircraft.

There has been an increase in respondents of DD Form 2627 and 2628 from 42 to 50 respondents. Respondent burden has increased to $72,900. DD Form 3062 has been added to the collection to replace DCMA Form 644, but it is an identical in format. This has increased the public respondent burden by $70,200. This has also increased cost to the Federal Government by $18,096.
