Short Statement: This information collection is currently approved with 4,415 burden hours and 33,664 responses. FNS has recently extended this study to collect information concerning dietary practices and health and nutrition-related outcomes from participants who are or will be turning 9 years of age. In preparation for this extension, FNS requests approval to add the previously approved contact information form back into this collection and to send birthday cards to the respondents and to their children when they turn 7 and 8 years old. FNS estimates that 1,149 burden hours and 18,432 responses will be added to the collection as a result of this program change. With this request, FNS estimates that this collection will have 52,076 responses and 5,564 burden hours.
