Abstract:
This submission is to make a non-substantive change request to existing information collections pursuant to 44 U.S.C. § 3507. The Commission seeks to change the FCC Form 5640 and FCC Form 5641(OMB Control No. 3060-1270). Previously, the Federal Communications Commission (FCC) sought and obtained approved from the Office of Management and Budget for multiple information collections needed to implement the Secure and Trusted Communications Networks Reimbursement Program (Reimbursement Program), OMB Control No. 3060-1270. The various filings are submitted using the FCC Form 5640 and 5641. To further protect the Reimbursement Program from waste, fraud, and abuse and to align the certifications with other recently implemented funding programs by the FCC, the FCC’s Wireline Competition Bureau revised the certifications associated with the FCC Form 5640. See Wireline Competition Bureau Announces Best Practices for Equipment Disposal and Revises FCC Form 5640 Certifications for the Secure and Trusted Communications Networks Reimbursement Program, WC Docket No. 18-89, Public Notice, DA 21-1234 (WCB Sept. 30, 2021). The Commission revises this collection to implement the Supply Chain Reimbursement Program, and related requirements, for the reimbursement of expenses by providers of advanced communications services for the removal, replacement, and disposal of covered communications equipment and service that poses a national security risk.
