Short Statement:
• The mode of collection has been changed to 100% online, where previously one of the 75 respondents was approved to submit a Word survey. The IC for the mail-in survey has been deleted and the respondent has been added to the IC for the web survey. There is no change to the total number of respondents. A Word version of the survey is provided as an aid to reviewers who may prefer this format. • Changes to survey language are itemized in SSA.15. These changes include simplified language; modifications to be more inclusive to newly funded territorial programs; and other edits to improve clarity and to ease answering via the availability of standard responses and to capture additional relevant information. • Questions asking about BLLs were modified to include an answer option for the updated BLRV of 3.5 micrograms per deciliter. Additionally, questions about the updated BLRV were added to the survey.
