Short Statement: This is an extension without change (with changes in estimates) to a current information collection request (ICR). The current OMB inventory for this ICR shows a total burden of 11 hours and 110 responses, while the requesting inventory estimates a total burden of 18 hours and 200 responses. Overall, the burden for this submission has increase by 7 hours and increased by 90 responses. Additionally, the cost to respondents under question 13 increased from $3,696 to $4,200. There is no change in the collection method. The increase in burden hours is solely the result of adjustments.
