Short Statement: This extension request includes updated templates from NCEH/ATSDR (Attachment 2c) and NCIPC (Attachment 2d). Also, due to a 2023 reorganization of Centers within CDC, CSTLTS has merged with another center and is now the National Center for State, Tribal, Local and Territorial Public Health Infrastructure and Workforce (NCSTLTPHIW); the new name is reflected on that template (Attachment 2b). The remaining templates for NCCDPHP (Attachment 2a) and NCBDDD (Attachment 2e) are unchanged. The burden is increased to 1450 hours.
