Short Statement: With this Revision, there was a decrease in the total estimated annual hour burden. This result stems from a decrease in the average burden per response from 0.75 hours to 0.58 hours due to the update including new condensed signature language in the Form and Instructions. There are no other program changes in this revision. There are no changes to the annual cost burden to respondents for this information collection.
