Abstract:
This series of forms are used to obtain information from the Person Authorized to Direct Disposition (PADD) or the Person Authorized to Effect Disposition (PAED) of remains of active-duty military, Reserve Component members, applicants, trainees, military prisoners, and others. These forms allow the Department to provide mortuary and transportation services as requested by the PADD or PAED. These forms also allow the Department to provide reimbursement, cover expenses, or otherwise provide mortuary services for decedents, including civilian employees serving with the armed forces. In order to provide reimbursement or these services, the Department requires notification by a PADD so that payment/reimbursement can be made.
