Short Statement: This is an extension to a previously approved request for the collection of information. The number of eligible respondents in the industry has decreased due to industry consolidation, which led to a decrease in the estimated annual burden hours from 14 to 3. This resulted in a decrease in annual costs to respondents and Federal Government.
