Abstract:
The information collections will provide DASH with data to generate internal reports that will identify successful and problematic surveillance and programmatic areas. In addition, both information collections will allow DASH to determine if recipient agencies are completing the required activities of the NOFO on time, as well as identifying problems in implementation. With this information, DASH can ascertain if additional technical assistance is needed to help recipients improve their surveillance or program implementation if necessary.

Short Statement:
This request consists of adding a single question to the survey and extending the data collection for an additional year. The following documents have been retired from OMB #0920-1329: Component 1 Work Plan - Survey; Component 2 Work Plan - Survey; and Component 2 & 3 Professional Development Work Plan - Survey. These survey forms were retired because the data no longer needs to be collected for NOFO PS18-1807. The last time these survey forms were used was April 1, 2023. The following documents are being revised and extended in the request: Component 1 – Surveillance Reporting Template – Work Plan Status Report - Survey and Component 2 – Programmatic Reporting Template – Work Plan Status Report - Survey. There are two remaining data collections for these two documents. They are due on April 1, 2024 and August 31, 2024. One question was added to the above two documents. The question is as follows: “(If applicable) Publications: List publications resulting from the project, as well as plans for further publications.”
