Abstract:
State disability determinations services collect the information SSA needs to administer our disability program. For the purposes of this ICR, we divide this information into three categories: 1) consultative examinations (a/b/c); 2) medical evidence of record; and 3) pain/other symptoms/impairment. Respondents are individuals, private sector, and State and local governments. We are submitting a Change Request to update the Telehealth Consultative Exam Agreement form with new information. The increased information on the form will also minorly increase the burden for the form.

Short Statement:
We are increasing the burden to the Telehealth Call Script and Agreement form due to implementation of SSA's emergency guidance to permit the use of alternative video technology to conduct video. The increase in the information the respondents must read will minorly increase the overall burden for this information collection.
