Terms of Clearance: By April 2025, USDA agreed to have updated this form to reflect Statistical Policy Directive No. 15: Standards for Maintaining, Collecting, and Presenting Federal Data on Race and Ethnicity (SPD 15). If that is the only change to this collection, USDA can submit a non-substantive change to achieve this. If there are additional edits that go beyond SPD 15, USDA should issue a new 60 day notice for revision by April 2025.
