A written request for Federal assistance may be submitted on a Resource Request Form (RRF). The RRF is a working document requesting Federal assistance and may be sourced as a Mission Assignment. The Mission Assignment (MA) form is used to record a request for Federal assistance by States/Tribes/Territories to FEMA, and is the official FEMA obligating document that tasks another Federal agency to perform work.

Short Statement:
The total reduction with this revision is 4,873 burden hours. Usability testing of FEMA Form FF-104-FY-21-121 (formerly 010-0-8a), MATO Form accounts for the first 1,067 hours reduced due to the reduction of the average burden per response by ten minutes (0.1667 hours) (6,400 total responses x 0.1667 hours = 1,067 burden hours). The rest of the reduction is based on data analysis of the average number of respondents and responses for the past ten years for each collection instrument, which led to a large reduction in the total number of responses, and is offset by an increase in training offerings. The increase in costs stems from the general increase in wages.
