Short Statement: This revision reflects an overall lower annual burden estimate of 18.06 hours, which is a decrease from the current estimate of 18.37 hours, due to FNS no longer accepting interventions via this form. This revised collection is estimated to receive a total of 43 responses per year, which is a decrease from the current estimate of 110. The changes for these estimates are due to assumptions that this collection will receive less responses overall as interventions are no longer being accepted through this form. The estimated time per response for this voluntary collection is 0.42 hours (25 minutes), which is an increase from the current estimated time per response of 0.167 hours (10 minutes). The estimated increase in time per response is due to the change of submitting relevant attachments by email instead of through the database. The estimate is also based on the length of time to read the form instructions and respond to the survey due to changes made to the form itself.
