Abstract:
This is a request for nonmaterial/non-substantive changes to clarify the meaning/intent of questions in both the Five Year Regional Workplace template and the Cooperative Agreement Workplan template.. The nonmaterial/non-substantive changes should not affect or lessen the burden in completing the forms. As some of the questions on both template were replaced with others; edited for clarity; or removed. In addition, the forms were transferred from WORD documents to a REDCap-based format, for ease of data entry by the respondents and collection by CDC’s program office. The REDCap-based form can easily export data either into EXCEL or PDF formats for comprehensive analysis by the program. There is no change in burden.

On behalf of this Federal agency, I certify that the collection of information encompassed by this request complies with 5 CFR 1320.9 and the related provisions of 5 CFR 1320.8(b)(3).
The following is a summary of the topics, regarding the proposed collection of information, that the certification covers:
