Short Statement: This is a reinstatement, with change, of a previously approved collection for which approval has expired. As a result of the changes described below, the number of estimated respondents decreased from 27,384 to 21,765 (a reduction of 5,619), and the annual burden hours decreased from 25,638 to 10,501 (a reduction of 8,983).
