Abstract:
This collection of information requires respondents to file annual reports with the Commission demonstrating the coverage provided with support from the Mobility Fund for five years after qualifying for support. Documentation prepared for, or in connection with, the award of Mobility Fund Phase I support is to be retained for a period of not less than 10 years after it receives the final disbursement of support. The Commission created new FCC Form 690 to ensure that recipients of Mobility Fund Phase I support are meeting their obligations for receiving support. The FCC Form 690, Annual Report for Mobility Fund Phase I Support, format is being modified as a result of the development of online electronic filings by both the Federal Communications Commission and Universal Service Administrative Company.

Short Statement:
The Commission is reporting an adjustment in the estimated number of respondents that will file annual reports on FCC Form 690 each year from 34 respondents to 5 respondents and is reporting a reduction in the estimated number of annual responses for the annual report filing on FCC Form 690 from 880 responses to 30 responses. Notwithstanding this adjustment, the cumulative total number of annual respondents for this information collection remains 34 in order to account for the total number of respondents that must continue to retain records pursuant to the record retention requirement. However, the Commission is reporting the following overall program changes for this collection: a reduction in the total number of annual responses from 880 responses to 64 responses (-816 responses), and a reduction in the total annual hour burden from 15,874 hours to 574 hours (-15,300 hours).
