Short Statement: The form in this collection is used to request a substitute savings bonds purchased in paper form. The sale of paper savings bonds ceased in December 2012 and the exception to purchase paper bonds with a tax refund was discontinued in December 2024. The decline in the use of the form reported in Item 12 is proportionate to the amount of paper savings bonds issued. The reduction of 17 burden hours is a program change due to agency discretion for a total of 150 hours requested.
