From the last submission to OMB, the Commission is reporting adjustments/decreases to this information collection. These adjustments are due to a decrease in the number of intake forms submitted which resulted in a reduction to the total number of respondents by -3 respondents, the total annual responses by -3 responses, and the total annual burden hours by -9 hours. The total annual cost remained the same. There are no program changes to this information collection.

On behalf of this Federal agency, I certify that the collection of information encompassed by this request complies with 5 CFR 1320.9 and the related provisions of 5 CFR 1320.8(b)(3).
