Short Statement: This is a request for an extension of a previously approved form with revisions to align directly with the requirements in 2 CFR 200.231, improve the function of the form, and reduce recipient burden. Revisions include a removal of four of the questions on the form. Burden estimates have reduced significantly from 12,000 annual hours to 1,320 hours.
